last name, first name, patronymic (the latter if available) or name of the insurance agent, insurance broker (search is performed without quotation marks);
taxpayer identification number (if available);
number of the agreement concluded between the insurer and the insurance agent, insurance broker;<
domain name and (or) network address of the official website of the insurance agent (if available), insurance broker in the information and telecommunications network "Internet".
