Requirements for the execution of medical documents

An extract from the outpatient card or discharge summary must be provided in the original and have a stamp of the medical institution, signature of the chief physician/head of the medical institution, seal, and must contain the following information: full name and age of the patient, date, time and place of the event, circumstances of the event, diagnosis, period of treatment, results of diagnostic tests and treatment performed, consequences, information on issued sick leaves.

If a copy of outpatient card is provided, it must be duly certified (pages, including the title page, are stitched, numbered, certified by the seal of the medical institution and the signature of the head physician/head of the medical institution). Uncertified copies of outpatient cards will not be accepted for consideration.

If a x-ray examination was performed, it is necessary to provide a conclusion on the results of the examination. The images are provided by the client in the event of an additional request from the insurance company. The results of other studies are provided in the same way.

To confirm temporary disability, it is necessary to provide a copy of the sick leave certificate, certified by the company's HR department. The sick leave certificate must be issued in accordance with Russian legislation. Only closed sick leave certificates with the date of return to work will be accepted for consideration (if the sick leave certificate is not closed and does not have a stamp or indication of the medical institution where it was issued, such a sick leave certificate will not be accepted for consideration.

Requirements for identity documents

It is necessary to provide a copy of the insured person's passport (including the page indicating the place of registration). If a child is insured, a copy of the child's birth certificate and a copy of the applicant's (parent/guardian's) passport must be attached to the Application).

Requirements for some other documents

In the event of an accident, a certified copy of the traffic accident certificate with the name of the victims, stamped and signed by a traffic police officer, must be provided.
